Club Registration Form

In order to ensure that our insurance policy will give you the cover required we need to establish some basic facts. It is important that you ensure that any information you provide is accurate and complete to the best of your knowledge.

The club instructor is the main contact and person of responsibility for the club. As such the club registration fee of £30, in addition to providing Public Liability Cover (inclusive of Member to Member) for the club itself, provides them with the following at no additional cost: Membership of the Association, Personal Accident Cover & Professional Indemnity cover for their year of membership.

If you have additional instructors they must be members of the Association. In addition they can be registered as an instructor for a yearly fee of £20/instructor below or here. This will give them Professional/Instructor Indemnity Cover.

Do you maintain an incident log book

Do you have an appointed first aider

Do you have first aid equipment available at your club

Have you got risk assessments in place where applicable

If you teach children do you have an emergency procedure for missing children

Do you use weapons and if so what? Please list in the box below.

Please Note Live bladed i.e sharp weapons are not covered by our Insurance Policy

List of Your Clubs

A club is defined as being in a different venue, so if you run classes on different days in the same premises that is one club.

Additional Clubs run by you can be registered for an additional £15/club.
